The same height chart, the same posture, two different Vitalis. The frame on the left was taken at the start of the process; the one on the right when lengthening was complete. The difference is exactly 10 centimetres — the result of a quadrilateral plan in which the femurs and tibias were lengthened on the same calendar.

Some journeys do not begin at an airport, in a hospital room or in an operating theatre. Sometimes everything starts with a single e-mail sent from another corner of the world.

That is how Vitali’s limb lengthening journey began.

When he first contacted us, his plan was not yet as clear as it is today. He wanted to be taller, but he was still weighing his options: which bone, which method, and in how many stages. His first idea was to lengthen a single segment. Later, he began considering lengthening the femurs and the tibias at different times.

After that first e-mail, the conversation kept going. In time he reached us on Telegram, and WhatsApp became part of our exchanges as well. Vitali was someone who liked to ask questions, wanted the details, and compared his options before deciding. So our conversations never stayed limited to price or the number of centimetres. From the available methods to the recovery process, from the time he would spend in Istanbul to physiotherapy, we went through many aspects of the journey together.

And Vitali had one more important thing to solve: time.

The period he could stay in Türkiye was roughly two months.

So even before the surgery took place, his story began to take shape around two goals. On one side was the height gain he wanted. On the other was a calendar that never stopped moving.

CHAPTER 01From a Single-Segment Plan to a Quadrilateral Decision

Vitali’s first plan was not quadrilateral lengthening.

In the beginning he was thinking of lengthening only one segment. As his research progressed, he started to consider lengthening both the femurs and the tibias. For a while, the plan in his mind was to do these two procedures at different times: have one segment operated on first, recover, and then plan a second surgery for the other segment.

But in limb lengthening surgery, planning is never done on the target centimetres alone. The current bone lengths, body proportions, the condition of the muscles and joints, the total amount of lengthening being aimed for, the method to be chosen and the rehabilitation process are all evaluated together.

Vitali’s goals were assessed from exactly this angle during his consultations with the surgeon.

Different options were discussed. Vitali asked his questions, the surgeon shared his medical assessment, and the plan — which had changed shape several times since that first e-mail — finally became clear.

Vitali decided to begin quadrilateral limb lengthening with the FITBONE method.

And so a surgery originally imagined for a single segment turned into a much more comprehensive journey, in which both the femurs and the tibias were followed within the same overall treatment plan.

VITALI’S VLOG — THE BEGINNING

CHAPTER 02What Is Quadrilateral Limb Lengthening?

Quadrilateral limb lengthening refers to lengthening the femur and tibia bones of both legs within the same overall treatment process. Instead of only two femurs or only two tibias, a total of four bones are included in the treatment plan.

For this reason, quadrilateral lengthening does not simply mean aiming for more centimetres. It is a more comprehensive process in which the lengthening of four bones, the formation of new bone, the movement of muscles and joints and the rehabilitation programme are all followed together.

For Vitali, this had a very concrete meaning in daily life.

On check-up days, attention was never on a single segment. The femurs and the tibias were followed together. The physiotherapy programme ran accordingly, and the lengthening sessions were spread across specific hours of the day.

When the quadrilateral decision was made, it was not only the surgical method that had been chosen.

The daily rhythm of the weeks ahead of Vitali had begun to take shape as well.

CHAPTER 03Why Did Vitali Choose the FITBONE Quadrilateral Method?

Following the surgeon’s assessment and the consultations, it was decided that FITBONE would be used in Vitali’s treatment plan.

FITBONE works with an internal lengthening system placed inside the bone. In Vitali’s plan, both the femur and the tibia segments were handled within this approach.

Once the decision was made, the conversations moved from assumptions to real preparations. Pre-operative tests, the length of the stay in Istanbul, the operation, physiotherapy and the lengthening programme that would follow were planned one by one.

And in the middle of all these serious preparations, Vitali’s character made itself known right away.

He was a businessman. He spoke English and Russian. He was highly social, talkative, witty and positive. He followed his treatment with real seriousness, but he had no intention of turning his life into a schedule that revolved only around surgery.

That attitude would become even more visible after the operation.

CHAPTER 04Surgery Day: The Moment the Plan Became Real

After long consultations and preparations, the day of surgery arrived.

For Vitali, this was the day a plan discussed for months finally began in the most concrete way.

Yet in limb lengthening, completing the operation does not mean completing the journey. The surgery marks the beginning of the controlled lengthening and rehabilitation period.

The first phase of recovery began right after the operation. Wound checks, mobility, physiotherapy and the lengthening programme that would start in the next stage were now going to be part of Vitali’s everyday life.

As a quadrilateral patient, both his femurs and his tibias had to be followed within the same process.

CHAPTER 05The First Days After FITBONE: From Dressings to Daily Life

In the first days after surgery, the body needs to adapt to its new situation. Many new routines begin in this period, from the first movements to physiotherapy. For his first steps in mobility and to settle into the process, Vitali stayed in the hospital under the surgeon’s supervision for 7 days. With the surgeon’s approval, he was discharged after those 7 days and transferred to his apartment by ambulance, where his follow-up continued under the care of the treating medical team.

Because FITBONE is a fully internal system, the areas requiring dressing care in Vitali’s case were followed up quickly. Once his wounds were in suitable condition and the medical team had given their approval, he was able to start showering.

In a long rehabilitation process, taking a shower or doing some of your daily routines on your own again may look like small details. For the patient, however, these are important milestones that make you feel daily life is being rebuilt.

Vitali’s mobility also allowed him to keep up his daily routines within his own recovery. He went back and forth to the exercise room for his physiotherapy sessions, and did as much as he could by himself.

Of course, every patient’s recovery is different. Vitali’s experience reflects his own individual treatment journey.

CHAPTER 06How Did Vitali’s Quadrilateral Lengthening Programme Progress?

In limb lengthening, change does not happen overnight.

After the lengthening line has been created surgically, a gradual lengthening period begins according to the programme set by the surgeon. Small daily changes accumulate over time to form the total lengthening being aimed for.

Vitali’s quadrilateral lengthening programme was divided into three different times of day.

At 9:00 AM both the lower and upper legs, at 4:00 PM only the upper legs, and at midnight both the lower and upper legs again — that was the programme he followed.

Before long, these hours became the fixed points of Vitali’s day.

The morning session, physiotherapy and the other routines of the day, the afternoon application and the final lengthening at midnight…

What was a new routine to get used to in the first days became a natural part of everyday life as the weeks went by.

“Okay bro, my current lengthening program is this: 9 am both lower and upper legs, 4 pm only upper legs, midnight both lower and upper legs. It’s better to do this at 9 am, because you’ll see both legs.” — Vitali

It should also be noted that Vitali’s programme was specific to his own treatment. The amount of lengthening and the frequency of application can vary according to each patient’s individual treatment plan, as determined by the surgeon.

CHAPTER 07A Businessman Who Tracked His Lengthening in an Excel Sheet

One of the most characteristic details of Vitali’s story was the way he tracked his own progress.

He carried the planning habits of his business life into his treatment.

He built his own Excel spreadsheet.

He recorded the lengthening amounts, marked off the days and followed his progress in numbers.

Every day there was a small value added to the table. On their own, these figures might not look like much. But as days turned into weeks, the numbers in the Excel file began to add up.

The centimetres that had once been only a target in our conversations were now turning into real, day-by-day recorded progress.

On one side was Vitali’s Excel sheet.

On the other was the bone formation the surgeon was following through X-rays.

This picture said a lot about Vitali’s character. Behind the witty, easy-going manner was a disciplined patient who followed his treatment with remarkable order.

CHAPTER 08Four Separate X-Ray Checks in Quadrilateral Lengthening

Following four bones in quadrilateral treatment was reflected in Vitali’s check-up days too.

Because the femurs and the tibias were evaluated separately, X-ray images of four regions were followed.

The daily progress in the Excel sheet was not enough on its own. The condition of the newly forming bone and the development of the lengthening sites had to be evaluated by the surgeon on the images.

So two different tracking systems ran side by side in Vitali’s treatment.

Vitali followed his daily progress in numbers.

The surgeon evaluated what that same process looked like inside the bone, on the X-rays.

CHAPTER 09Physiotherapy and Mobility After Quadrilateral Lengthening

In limb lengthening, it is not only the bones that are followed. As bone length changes, the surrounding muscles and other soft tissues also need to adapt to the new situation.

That is why physiotherapy became one of the core parts of Vitali’s daily programme.

He followed his sessions regularly. Within his own recovery, his mobility was at a level that let him keep up his daily routines, and he could go to and from his physiotherapy sessions.

After a while, the treatment programme and daily life stopped being two separate worlds.

There was the morning lengthening session.

Then came physiotherapy and the day’s errands.

In the afternoon, the second application.

In the evening, time was left for rest, a film or music.

And at midnight, that day’s lengthening programme was completed.

This routine continued for weeks.

CHAPTER 10Two Months in Türkiye: Vitali’s Race Against the Calendar

In Vitali’s quadrilateral story, what mattered was not only how much the bones lengthened, but also how many days were left on the calendar.

He could stay in Türkiye for roughly two months.

For a process spread over weeks like quadrilateral lengthening, this made planning all the more important. As Vitali’s lengthening programme progressed, his legal period of stay in Türkiye was shrinking at the same time.

So the dates in his Excel sheet did not only show which day of treatment he was on. They also reminded him how long he had until the day he had to leave the country.

Alongside physiotherapy, X-ray checks and the lengthening programme, the return plan had to be tracked as well.

At the end of the planned process, Vitali left Türkiye. Because of the treatment timeline, however, he stayed 7 days longer than planned. On departure, he paid an overstay penalty of roughly 1,300–1,900 Turkish lira for the extra days.

This detail was a real-life reminder that, in a long treatment abroad, planning the surgery date alone is not enough. Accommodation, flights and the legal period of stay all need to be considered together with the treatment calendar.

For Vitali, it was time to leave Istanbul.

But his story would not end there.

CHAPTER 11Life in Istanbul: Turkish Words, Burgers and Sunflower Seeds

Although the treatment was intense, Vitali did not spend his life in Istanbul only between physiotherapy and X-rays.

His social nature showed itself in this period as well.

Vitali, who speaks English and Russian, began trying to learn Turkish words while he was in Istanbul. He used the words he had just learned in his conversations with our team.

Sometimes he found the right word; sometimes the result was an expression that made everyone laugh.

A SMALL CELEBRATION

Cake, walkers and a room full of people

Fellow patients, coordinators and physiotherapists squeezed into one apartment living room for a cake. For Vitali, moments like this were the other half of the treatment: the part that does not show up on an X-ray.

Over time, these small moments became a natural part of the communication between Vitali and the team.

When it came to food, he built his own list of Istanbul favourites too.

He loved burgers. Pizza and sunflower seeds also found their way onto his list of favourites.

On one side, there was a disciplined patient following four separate X-rays and logging his centimetres in Excel.

On the other, a sociable Vitali trying to learn Turkish, eating pizza and cracking sunflower seeds.

They were, of course, the same person.

CHAPTER 12Films, the Guitar and the Turkish Songs He Learned

In a long treatment, how you spend your free time is an important part of daily life.

Vitali watched a great many films during the process.

But he did not spend his time only in front of a screen.

He loved playing the guitar, and music became one of his Istanbul routines. Before long, his curiosity about Turkish merged with music.

He began learning Turkish songs.

He had set himself a playful goal: to play the songs he learned on his guitar and, in the end, give a small concert.

The result was a rather unusual daily routine.

In the morning, Vitali carried out his femur and tibia lengthening programme; during the day he went to physiotherapy; in his free time he played the guitar and learned new Turkish songs; and at midnight he completed the day’s final lengthening session.

While the treatment went on, life went on too.

CHAPTER 13Days in Istanbul and Social Activities

Vitali came to Istanbul for surgery, but over time the city became more than just the place where he was being treated.

When his treatment programme allowed, he went out and joined social activities. In the time he spent with our team, his witty and talkative character showed itself immediately.

Some of these moments were caught on camera.

In these clips there is a side of Vitali that is different from the one in the hospital setting — and, in fact, describes him far better.

Someone laughing, talking, joking and trying to spend his time in Istanbul as much as possible inside ordinary, everyday life.

ISTANBUL — EXCURSION AND EVERYDAY MOMENTS

CHAPTER 14The New Request 24 Days After Leaving Istanbul: 2.5 cm More on the Tibias

Leaving Türkiye did not mean Vitali had put the centimetre count entirely out of his mind.

Twenty-four days after returning home, he got in touch with our team again.

This time he had a new goal in mind.

He wanted to lengthen his tibias by another 2.5 cm.

Since the FITBONE system was still in Vitali’s body, one might assume at first glance that the existing lengthening could simply be resumed where it left off. But what determines whether lengthening can continue is not only whether the implant is still inside the bone. The healing stage of the new bone that has formed along the lengthening line matters just as much.

Vitali’s request was conveyed to the surgeon and his current condition was evaluated.

In the time that had passed, bone consolidation in the tibia region had progressed.

For that reason, it was not possible to continue lengthening by another 2.5 cm along the existing line simply by reusing the device.

The surgeon explained that further lengthening would require a new osteotomy — a new bone-cut line. In other words, the additional 2.5 cm could no longer be carried out as a simple continuation of the existing treatment. It would need a new surgical procedure and fresh medical planning.

This created an interesting turning point in Vitali’s story.

The new bone we had watched forming on the X-rays for weeks had now begun to consolidate and grow stronger. This expected progress in healing also meant that the window for additional lengthening along the existing line had closed.

The lengthening figures in Vitali’s Excel sheet had stopped, but the healing inside the bone continued.

This experience also shows why the target amount in limb lengthening needs to be planned carefully from the start. A patient may change their goal in the later stages of treatment, but whether additional lengthening is possible is not simply a matter of wishing for it. Bone formation, the level of consolidation and the current clinical picture must be re-evaluated by the surgeon.

CHAPTER 15Quadrilateral Lengthening Is Not Just Surgery Day

Seen from the outside, limb lengthening is usually told through two points: the starting height and the height reached.

But between those two points lie weeks and months.

In Vitali’s story, that space was remarkably colourful.

One day there was an X-ray check.

Another day, a physiotherapy session.

At 9:00 AM the lengthening programme ran, at 4:00 PM it continued, and at midnight the last application of the day was done.

On some days, new values were added to the Excel sheet.

On some evenings, a film was watched.

Sometimes the guitar was played, sometimes a new Turkish word was learned, and sometimes an ordinary day in Istanbul turned into one of the treatment’s most memorable moments.

That is exactly what made Vitali’s quadrilateral journey so interesting.

For him, the process was never only the controlled lengthening of four bones. It also became two months spent in another country, a calendar that was constantly checked, daily discipline, new people and goals that changed along the way.

On the day he sent that first e-mail, he had no idea how any of this would unfold.

An idea that started with a single segment turned first into a femur-and-tibia plan and then into a FITBONE quadrilateral surgery. The lengthening process that began in Istanbul came back onto the agenda, with a new 2.5 cm request, even after he had returned home.

This is why Vitali’s story is not a story of change that moves along a flawless, straight line.

It is a real quadrilateral limb lengthening experience in which decisions, check-ups, changing goals, discipline and everyday life are woven together.

Frequently Asked Questions About Quadrilateral Limb Lengthening

What is quadrilateral limb lengthening?
Quadrilateral limb lengthening is the lengthening of the femur and tibia bones of both legs within the same overall treatment plan, so that a total of four bones are included in the treatment and follow-up process. Suitability and the amount of lengthening that can be applied are determined by each patient’s individual evaluation.
Can the femur and tibia be lengthened with FITBONE?
FITBONE is an internal lengthening system. In Vitali’s treatment, a quadrilateral plan covering the femurs and tibias was applied following the surgeon’s evaluation. It cannot be assumed that every patient is suited to the same treatment; the choice of method should be made after the surgeon’s assessment.
Why are four X-rays taken in quadrilateral lengthening?
Because the femur and tibia regions need to be followed separately, images of four regions were evaluated during Vitali’s quadrilateral process. X-ray checks are used to assess new bone formation and consolidation along the lengthening line.
Can lengthening be repeated after it has been completed?
This depends on the current healing stage of the bone. Twenty-four days after returning home, Vitali requested another 2.5 cm on his tibias. However, as the surgeon’s evaluation showed that bone consolidation had progressed, continuing to lengthen along the existing osteotomy line was found not to be possible. If further lengthening is considered, new surgical planning may be required.
Why is physiotherapy important during quadrilateral lengthening?
As the bones lengthen, the muscles and other soft tissues must adapt to the changing length. That is why physiotherapy and regular monitoring of joint range of motion are important parts of the rehabilitation process. The programme is planned by the medical team according to the patient’s condition.
